Patient Registration

To register as a patient, please –

• Ensure you are living within our boundary area – in general terms, the town of Kirkcaldy only

• Complete a registration/application form for every person wishing to register, as well as a New Patient Questionnaire or Under 5s Questionnaire.  You can either download and print these forms, or collect them from our reception desk.

• Make sure you have the correct ID for everyone, where appropriate.  Photo ID and proof of address dated within the last three months.

• Once you have all of the above information prepared, please hand these in at our reception desk.
